Medical Forms

 If you require medicine to be administered to your child, permission must be given by completing this form.

Leave of Abence Form

Fill this form in if you are planning to take your child out of school for any reason.

Notification of Appointment Absence Form

This form is to notify the school that you may be collecting your child early for an appointment or arriving at school later on. This form also can be used if your child has to take the whole day off due to an appointment.  

Proof of appointment is required.
